Immediate provisional restoration of single-piece zirconia implants- a prospective study – results after 18 month of clinical function

Ceramic implants are documented since 40 years. For the last years it is questionable whether zirconia dental implants could be a serious alternative to routinely used and well documented titanium dental implants. The white color is a proclaimed advantage in regions of aesthetic priority. Furthermore there are publications showing that zirconia is less susceptible to bacterial accumulation compared to titanium. This could mean that the incidence of biological complications.Aim of this prospective case series was to evaluate the outcome of immediately provisionalized single-piece zirconia implants.In the present clinical study 13 one piece zirconia implants were inserted in single-tooth gaps of 20 patients. Implants were restored with all-ceramic CAD CAM provisionals without occlusal contacts immediately after placement. Permanent all-ceramic restoration was performed 4 months after surgery. Plaque index (PI), bleeding on probing (BOP), pink aesthetic score (PES), mean radiographic marginal bone levels (MBL), implant survival and success were evaluated up to 18 months.Assessment of PI at baseline and follow-ups after 3, 6, 12,15 and 18 months revealed 31.5 (±18.6), 29.4 (±15.4), 36.9 (±20.03), 38.9 (±18.2) and 32.0 (±14.0), respectively. Evaluation of BOP revealed 15.5 (±11.6), 22.9 (±15.2), 17.9 (±12.5), 28.1 (±14.0) and 24.4(±15.2), respectively. PES improved from 7.71 (±1.5) at baseline to 10.82 (±1.33) 18 months after implantation. Measurements of MBL showed bone loss of 0.77 mm within the first year after and 0.99 mm 18 months post-implant insertion.The aesthetic and functional outcomes where highly satisfying. Also the**osseointegration and biocompatibility was comparable to the results of other publications.**However there are long term investigations necessary, before the routinely usage of this zirconia in dental implants can be recommended.****
